From 1 January 2026, Sunbury and Cobaw Community Health and Nexus Primary Health will merge to form Omnia Community Health – a unified, not-for-profit organisation delivering inclusive, person-centred care across the Hume, Macedon Ranges, Mitchell, Murrindindi and Strathbogie regions.
About the Role
Are you an experienced and passionate NDIS professional who believes in consumer-directed care and helping participants achieve their goals? Join our NDIS Support Coordination program and make a meaningful impact. We are looking for a motivated and experienced Support Coordinator to join our Community Connections Unit.
- Support individuals with diverse needs using person-directed care principles
- Strong communication, planning, and problem-solving skills are essential
- Some travel between sites may be required

About You
The ideal candidate will have:
- Demonstrated knowledge and understanding of the NDIS
- Demonstrated ability to develop resources and systems to support better outcomes for participants with complex support needs
- Experience in a relevant role, preferably the disability, health or mental health sectors
- Demonstrated ability to build and maintain professional networks
- Demonstrated evidence of excellent time management and organisational skills, and the ability to prioritise tasks
- Understanding and demonstrated commitment to the social model of health
