A Home Care Package is a program of the Australian Government that provides money to buy services for eligible older people to live well and safely at home and in the community.

As a Home Care Package Provider, we can organise help with daily living and healthcare as part of a Home Care Package, including:

With a Home Care Package, you have choice over what services you get, where and when you get them and who you get them from.

Home Care Package Service Providers

All service providers contracted by Sunbury and Cobaw Community Health to support our Home Care Package clients are required to provide us with documentation to ensure compliance with the Aged Care Act 1997.
