We have a number of services that support adults of all abilities to live healthy, active and connected lives.

Locations

We provide services to people in Sunbury and the Macedon Ranges.  Details of where each service is delivered is available on the relevant service website page.

Funding your care

The cost of services can depend on:

  • Your income
  • If you have a concession card
  • If you are eligible for government funding and which kind
  • What services you need

Some services are free. We will let you know the cost of any other services and appointments.

These are some of the ways people can use and pay for our services:

Commonwealth Home Support Program (CHSP)

For people who need services to support them to live at home. There will be a small cost.

Home Care Packages

For people over 65 years of age who need a higher level of support than CHSP to live at home. Eligible people can get funding to use on services to help them live well and safely. The cost of our services will be covered by your package so you will not be out of pocket.  To find out more about Home Care Packages through Sunbury and Cobaw Community Health, click here.

If you have a Home Care Package with another provider you can still purchase services from Sunbury and Cobaw Community Health if we have a current service agreement with your provider.  Click here to see the services available and the associated fees.

NDIS (National Disability Insurance Scheme)

For people with an NDIS plan who need services to help achieve their goals. The cost of services will be covered by your NDIS funding so you will not be out of pocket. Find out more here.

My Aged Care

If you are 65 years of age or over, before you can use some of our services you will need to register with My Aged Care. My Aged Care is a government resource where people 65+ can find services and information. The staff will talk to you about what to do so that you can choose the supports and services that can help.
