Accessing Your Records

You have the right to request access to your information and ask for it to be corrected if necessary.

You will need to download and complete a Request for Release of Client Records form, also available from any of our receptions.  We may charge a fee for photocopying and sending the information.  Our fee structure aligns to the Health Records Act 2021, which can be viewed on the Health Complaint Commissioner website.

If your request is made electronically, we will need you to provide a certified copy of your identification before we can release your records.  If you submit your request at one of our sites, you can show your identification to staff when submitting the form.

If you wish to apply for access to the health records of a person who is deceased, please complete a Request for Release of Client Records form and provide legal evidence of your relationship to the deceased.

Family Violence and Child Information Sharing Scheme

Where necessary, Sunbury and Cobaw Community Health will share information with other authorised organisations under the Family Violence Information Sharing Scheme and the Child Information Sharing Scheme.

If you are an authorised information sharing entity and wish to share information or make a request for information from Sunbury and Cobaw Community Health, please send through a completed Information Sharing Scheme request form.
