Sunbury and Cobaw Community Health is a not-for-profit community health organisation providing a comprehensive range of free and low-cost health, wellbeing and community services in early childhood, youth, families, adults, aged and disability.
Covering an area of over 1700 km2 from Sunbury to beyond the borders of Macedon Ranges Shire our 220 staff and 70 volunteers are based across four sites at Kyneton, Romsey and Sunbury.

About the Role
We’re looking for a passionate and strategic leader to join our Community Care Division as Manager Support at Home.
This role leads a multidisciplinary team delivering aged care services that help older people live independently, stay connected, and thrive in their communities.
What You Will Do:
- Lead and grow the Support at Home program (formerly Home Care Packages)
- Drive service excellence, innovation, and expansion into new regions
- Build strong partnerships across sectors and communities
- Manage budgets, performance, compliance, and quality assurance
- Mentor and support team leaders and staff across multiple sites

About You
The ideal candidate will have:
- Proven leadership in aged care, community services or similar sectors
- Deep understanding of Support at Home guidelines and aged care standards
- Commitment to inclusive, person-centred care
- Strategic thinking and business development skills
- Strong communication and stakeholder engagement abilities
