Following the recent announcement that Sunbury and Cobaw Community Health and Nexus Primary Health will merge, we are excited to begin developing a new brand identity and organisational values for our soon-to-be unified organisation.
We know how much both organisations mean to those that use our services, which is why we are inviting you to help shape the new identity and values.
This short survey seeks to understand your impressions of Sunbury and Cobaw Community Health and gather your hopes and expectations for the new entity.
Participation is voluntary, responses are anonymous, and your input will directly inform the name, branding, and values of the future organisation.
The survey will be open until 9am on Monday 28 July 2025.
