About Sunbury and Cobaw Community Health

Sunbury and Cobaw Community Health (SCCH) is in the Hume and Macedon Ranges government areas.  SCCH operates with a multidisciplinary team structure and employees are required to incorporate activities relating to health promotion, community consultation, early identification and intervention and individual and community capacity building within their role. Employees are expected to participate as a member of SCCH team and provide services within a social model of health.

Your New Team

  • Our experienced occupational therapy team provides the opportunity to work across a variety of age groups and funding bodies effectively delivering high quality services to our local population through the National Disability Insurance Scheme (NDIS), Home Care Packages and the Commonwealth Home Support Program funding.
  • We are committed to providing a high level of care within the community in a regional environment. We are committed to program development, innovation and working together as a team.
  • SCCH provides aged care and disability occupational therapy services across the Hume and Macedon Ranges government areas. We participate and contribute to specialist clinical programs including cardiac and pulmonary rehabilitation, falls prevention, and pain management.
  • SCCH is committed to delivering a high quality of care while supporting and encouraging professional development.

The successful person will have:

  • The ability to complete goal-directed care planning, case coordination, equipment and home modification prescription, and chronic illness management.
  • Excellent interpersonal skills and the ability to communicate effectively with clients, families, and carers as well as other service providers.
  • At least two years of clinical occupational therapy experience.
  • The willingness to work as a member of a multidisciplinary team.
  • A high level of written and verbal communication and interpersonal skills to operate in a collaborative manner within the organisation.
  • Knowledge and experience working with My Aged Care, State Wide Equipment Program and Geat 2 Go.
  • Knowledge and experience working under fee for service models such as the NDIS and/or Home Care Packages.
