Covering an area of over 1700 km2 from Sunbury to beyond the borders of Macedon Ranges Shire our 220 staff and 70 volunteers are based across four sites at Kyneton, Romsey and Sunbury.

About Nexus Primary Health

Nexus Primary Health is a not-for-profit community health organisation servicing clients across the Mitchell, Murrindindi and Strathbogie local government areas. Nexus Primary Health provides a range of services including National Disability Insurance Scheme support, allied health and GP services.  The Nexus story began over 40 years ago with a small local community health service employing a physiotherapist and district nurse.  Today they are a fully integrated primary health service with around 200 employees with sites at Wallan, Broadford, Seymour and Kinglake.

About the Role

The Merger Transition Manager will oversee all aspects of the merger through planning, transition and post-merger integration. The Manager will work collaboratively across the organisation, acting as a ‘champion’ for the merger and ensuring effective project management, stakeholder engagement, and change management.

The role provides strategic leadership for the merger process, working closely with the CEO, Executive team, and Board to ensure successful integration while maintaining business continuity.  This 12-month contract position will establish and lead a transition team dedicated to implementing the merger strategy and project plan.

Key Duties and Responsibilities

Merger Leadership
Support the strategic and operational alignment of merging entities. Facilitate collaboration and consistency across leadership teams during integration.

Integration Planning and Implementation
Develop and execute detailed integration plans across systems, processes, and teams. Monitor progress and adjust as needed to achieve a seamless transition.

Project Management
Coordinate and deliver key projects with clear timelines, budgets, and outcomes. Track progress, manage risks, and ensure successful delivery.

Change Management
Design and implement strategies to guide staff through organisational change. Support adoption through communication, training, and feedback loops.

Stakeholder Engagement
Build strong, collaborative relationships with internal and external stakeholders. Act as a key liaison to ensure transparency and address concerns.

Values and Governance
Promote organisational values and ethical practices throughout all activities. Ensure compliance with governance frameworks, policies, and legislative requirements.
