Seeking Service Providers
We are looking for more service providers to assist our Home Care Package participants.
A Home Care Package is a program of the Australian Government that provides money to buy services for eligible older people to live well and safely at home and in the community.
We are particularly looking for gardeners, cleaners and personal care providers.
All service providers contracted by Sunbury and Cobaw Community Health to support our Home Care Package clients are required to provide us with documentation to ensure compliance with the Aged Care Act 1997. For details of the requirements see the attached checklist.
