About the role:

This position is responsible for providing initial and annual assessments and goal directed care planning for adult clients and carers who are referred for Social Support and/or Respite services at Sunbury and Cobaw Community Health.

Clients live in the local community and may be referred via My Aged Care or may be participants of the National Disability Insurance Scheme (NDIS) or be holders of a Home Care Package.  This position will also be preparing programs of support documents for NDIS participants and manage the waiting list.

Additional duties include completing annual care plan reviews, updating personal support plans with current participants and providing administrative support for our 12 week activity programs.

The successful candidate will be required to undertake and maintain a National Criminal History Check (NCHC), an NDIS Safety Screening check and hold a current valid Working With Children Check.  As a healthcare provider, all our staff will need to be vaccinated for COVID-19 or have a medical exemption by an authorised medical practitioner.
