The Connection Service is for people who require assistance accessing and understanding services.
Who is the service for?
You might be eligible for the service if you are:
- Aboriginal or Torres Strait Islander
- Culturally and linguistically diverse
- Living in a rural or remote area
- Financially or socially disadvantaged
- Homeless or at risk of becoming homeless
- A parent separated from children by forced adoption or removal
- Living with dementia
- Lesbian, gay, bisexual, transgender or intersex
Our Connection Service is available to people living in the Macedon Ranges and is a free service.
It is for people aged 65 and under or aged 50 and under if you are Aboriginal and Torres Strait Islander.

The types of help we can give you
Connection Service can help you to:
- Find out what services are available to you
- Understand how a service works
- Understand the steps involved in applying for a service
- Think about how you would like the service to be provided
- Contact an assessment service for the Home and Community Care Program for Younger People
- Contact the National Disability Insurance Scheme (if you think that you may be eligible for this service)
- Think about any questions you have for the assessment process
- Contact Centrelink
We will work in partnership with you and others who support you (such as family members, friends, your community, your doctor and other health professionals) to enable you to confidently engage in appropriate services.

Connection Service for over 65s
We can support Macedon Ranges residents over 65 years who might need help with continence advisory services, dementia advisory services, and strategies to manage their conditions to remain living safely at home.
