Our Care Finder service can help you, or someone you know, access My Aged Care as well as find services especially for older people. It provides people with local, personalised and face-to-face support. The program has been created in response to a recommendation of the Royal Commission into Aged Care Quality and Safety.
Care Finder can:
- Support you with the My Aged Care assessment process
- Help you fill in forms
- Help you find health and care providers in your area
- Help you find supports and connections in your local community
- Provide assistance if you are at risk of homelessness
This service for people living in the Macedon Ranges and is free. If you live outside of the Macedon Ranges you can find your nearest Care Finder here.
Who is the service for?
Care Finder is for people who are eligible for aged care services (people aged over 65 years and Aboriginal and Torres Strait Islander people 50 years and over) and have one or more reasons for requiring intensive support to:
- Access My Aged Care
- Access aged care services
- Access other relevant supports in the community
