Who is Diabetes Education For?
Sunbury and Cobaw Community Health’s diabetes education service is provided by registered nurses and is for those over the age of 18 years with:
- Pre-diabetes
- Type 1 diabetes
- Type 2 diabetes
Diabetes education can help people manage their diabetes. Our diabetes educator can provide one-on-one support to people adapting to life with diabetes, by explaining what it is and how to manage it.
Don’t Sugar Coat Diabetes
We also offer an 8-week exercise and education group program called Don’t Sugar Coat Diabetes. The one-hour education sessions are hosted by a
different health professional each week. As well as our diabetes educator, you will hear from other health professionals:
- Exercise physiologist
- Podiatrist
- Pharmacist
- Optometrist and audiologist
- Dietician
- Cardiac rehabilitation nurse
We then spend the next hour in the gym where you will be guided through a personalised exercise plan drawn up by our exercise physiologist. Contact us to find out when the next group program is starting.
Cost
Give us a call to talk about the cost. This will depend on your income, if you have a concession card or if you use this service through funding such as a Home Care Package or the NDIS.
